Your 91制片厂 (ACPNY) Care Team includes nutritionists who are Registered Dietitian Nutritionists (RDNs) and Certified Diabetes Care and Education Specialists (CDCESs). As part of your total care, these providers offer one-on-one medical nutrition therapy, as well as diabetes self-management education and support.

Our RDNs can customize an eating and lifestyle plan for you based on your overall health profile, budget considerations, time limitations, and other emotional factors. In your work with our RDNs, you will receive:

  • Medical nutrition therapy, which includes current evidence-based guidelines.
  • Guidance on using food to improve overall health and manage chronic conditions like diabetes, cardiovascular disease, and kidney disease.
  • Help with developing tools for managing stress.
  • Strategies for maintaining healthy sleep and improving your fitness level through appropriate physical activity.

Diabetes Self-Management Education and Support

Our CDCESs can help with prediabetes and diabetes through education and self-care support. Covered topics include:

  • Which type of diabetes you have or are at risk of developing.
  • When to take diabetes medicine and how it works.
  • How food, medication, stress, and other lifestyle factors impact blood glucose.
  • Strategies for consistently achieving glycemic control.
  • How to use a blood glucose meter, continuous blood glucose monitor, and insulin pen or pump.
  • How to reduce risk and solve problems when challenges arise in your journey with diabetes.

Bariatric Surgery Preparation and Follow-Up

For patients undergoing bariatric surgery, our RDNs can work on intensive diet and lifestyle intervention for the six-month period prior to surgery, and then post-surgery follow-up to support sustainable weight loss through healthy diet and lifestyle maintenance. It is important to follow up with an RDN post-surgery to ensure you are meeting all your nutritional needs. The post-surgery diet consists of different advancement phases that our RDNs can oversee and help you with. Weight loss surgery is only one small piece of the puzzle in achieving long-term success, so it’s important that patients adopt a holistic approach to meal planning, stress and sleep management, and regular physical activity.

Certified Diabetes Care and Education Specialists (CDCESs) and/or diabetes educators who are Board Certified in Advanced Diabetes Management (BC-ADM?) work in an interactive and collaborative manner to educate and empower patients with diabetes to apply self-management skills. Their services not only benefit patients, but the physicians who oversee their medical care.
